Volunteer Registration

This form is to be completed by all Category One Volunteers.
Parent/Guardian Authorization for the Treatment of MinorsÌý
This form is to be completed by a parent or guardian of all Category One and Category Two volunteers who are minors.
Academic Service-Learning Risk WaiverÌý
This form is a risk waiver that must be signed by all FAU students completing an Academic Service-Learning project as part of their course.
